- Identification of the infringing material along with sufficient information for us to locate it (please provide the URL of the specific page in question);
- Sufficient contact information such as name, mailing address, email address, and telephone number;
- A statement that you have a good-faith belief that the use of the material is not authorized by the copyright owner, its agent, or the law;
- A statement that the information in the notice is accurate and that, under penalty of perjury, you are authorized to act on behalf of the copyright owner.
Please note that under 17 U.S.C. §512(f), you may be liable for damages (including costs and attorney’s fees) if you knowingly misrepresent that material or activity is infringing.

By submitting a complaint, you acknowledge and agree that we may forward your notice and contact details to the alleged infringer.
Counter-Notice – Restoring Removed Material
If you believe that material was removed by mistake or due to misidentification, you may send us a counter-notice.
Under 17 U.S.C. §512(g)(3), your counter-notice must include:
- Your physical or electronic signature;
- A description of the removed material and its original location before removal;
- A statement, under penalty of perjury, that you have a good-faith belief that the material was removed by mistake or misidentification;
- Your name, address, telephone number, and a statement that you consent to the jurisdiction of the appropriate federal court in your district (or any U.S. federal court if you are outside the U.S.) and that you will accept service of process from the person who submitted the original DMCA notice.
